## Short answer

In England, a standard NHS PPC costs £32.05 for three months or £114.50 for twelve months, checked on 5 November 2026. At £9.90 per chargeable item, it saves money from four items in three months or twelve items in a year. Check exemption eligibility, certificate dates and whether an HRT PPC better fits the items you receive.

An NHS prescription prepayment certificate (PPC) can reduce the cost of chargeable NHS prescription items in England. The useful comparison is the number of items you expect to pay for during its validity period, followed by a check that the certificate covers the right person and dates.

## Key points

- Compare the chargeable items collected within the covered period, rather than counting prescription forms.
- A standard PPC covers the named person’s NHS prescriptions; private purchases and dental treatment bills remain separate.
- Keep certificate validity, the payment schedule and any FP57 refund paperwork together.

## When does a PPC save money?

These are England’s prices, checked on 5 November 2026. An NHS prescription item costs £9.90; a standard PPC costs £32.05 for three months or £114.50 for twelve months. The government confirmed the 2026/27 freeze , and GOV.UK lists the certificate prices . Example within the covered period Pay per item PPC Saving 4 items in 3 months £39.60 £32.05 £7.55 12 items in 12 months £118.80 £114.50 £4.30 24 items in 12 months £237.60 £114.50 £123.10 These are calculations using the published prices. Count chargeable items, not prescription forms, pharmacy visits or tablets. Several medicines on one form can create several charges. Use expected collection dates; a yearly total does not show whether four items fall inside one three-month certificate.

## What is included, and what remains outside it?

A standard PPC covers your NHS prescriptions, including NHS dental prescriptions. It does not pay for private prescriptions, ordinary shop purchases or the dental treatment itself. The NHS explains the scope and item-based charging , including the exclusion of other health items such as wigs and fabric supports. The NHSBSA terms also exclude support tights and restrict use to your own NHS prescriptions. A certificate cannot be shared with a partner. Keep the medicine charge separate from any other service charge when reading a pharmacy bill, and ask the pharmacy which part is an NHS prescription item if the description is unclear.

## Check exemptions and the HRT alternative first

Before paying for a certificate, use NHSBSA’s eligibility checker . A benefit award or a health condition does not automatically settle every prescription exemption. The checker asks about the relevant circumstances and explains any next application step. An HRT PPC costs £19.80 for twelve months and covers only listed HRT medicines, regardless of why they are prescribed. Compare your actual mix of covered HRT and other chargeable items. For example, twelve qualifying HRT items plus four other items would cost £59.40 using an HRT PPC plus four £9.90 charges, compared with £114.50 for an annual standard PPC. This example assumes no other chargeable items; check the official medicine list before using it.

## Choose the dates and understand the payment arrangement

You can buy through NHSBSA online or at some pharmacies. A twelve-month certificate can be paid in ten monthly Direct Debit instalments through NHSBSA; that option is unavailable at pharmacies. Check the official purchase and payment routes . The start date can be up to one calendar month before or after purchase. Without a requested date, it starts on the purchase date. Read the confirmation rather than assuming the next collection is covered. Under the PPC terms , cancelling a Direct Debit can leave an outstanding balance. Instalments spread the price of the certificate; they are not a monthly cancellation arrangement. Keep the payment schedule beside the certificate expiry date.

## Validate the certificate before claiming

Use NHSBSA’s certificate checker to confirm a standard PPC is valid and see its expiry date. Match the name and details to the prescription, and contact NHSBSA about errors or a change of name or address. An old confirmation email is not evidence that the current collection falls inside the covered period. Set a reminder before expiry to review whether another certificate is needed and how renewal will be handled. Check the actual confirmation of any renewal. NHSBSA checks claims, and an incorrect claim can result in a penalty charge. The standard checker does not validate HRT PPCs; its page directs HRT certificate holders to the appropriate contact route.

## Keep the right receipt if you have already paid

If you pay a prescription charge that falls within your PPC dates, ask for an FP57 receipt and refund form when you pay . It cannot be issued later. NHSBSA says to claim the prescription refund within three months of payment. Follow its refund instructions and retain the certificate details with that form. A refund of the certificate’s purchase price is a different request. If you become exempt while a standard PPC is valid, a full or partial refund may be available; the terms require applying within three months of becoming entitled. This guide concerns England, where PPCs are available to residents. Scotland, Wales and Northern Ireland have different prescription arrangements; use our four-nations guide before applying England’s cost comparison elsewhere.

## Questions to ask

- How many chargeable NHS items do I expect to collect during the proposed three- or twelve-month period?
- Have I checked free-prescription eligibility and whether my HRT medicines qualify for the separate HRT PPC?
- Do the certificate name, start date and expiry date match the prescription and collection I am claiming for?
- If I paid a covered charge, did I obtain an FP57 at payment and record the refund deadline?

## Frequently asked questions

### Does a PPC cover NHS prescriptions from a dentist?

A standard PPC includes NHS dental prescriptions. It does not cover the dental appointment or treatment charge; those follow separate dental charging and exemption rules.

https://www.med-easy.co.uk/guides/nhs-prescription-prepayment-certificate/#faq-1

### Can I stop paying once my medicines change?

A change in medicines does not turn an annual PPC into a cancellable monthly plan. Contact NHSBSA before cancelling its Direct Debit because a balance may remain. Becoming exempt is a separate possible refund route.

https://www.med-easy.co.uk/guides/nhs-prescription-prepayment-certificate/#faq-2

### Can I check an HRT PPC with the standard exemption checker?

No. NHSBSA’s standard certificate checker excludes HRT PPCs. Use the HRT contact route linked from that page and check that the medicine itself is covered.
