## Short answer

NHS Mounjaro access depends on the nation, clinical pathway and current local rollout. In England, phased primary-care priority groups are narrower than the broader NICE recommendation. Ask your GP which route applies, whether it is accepting referrals and what evidence is needed. A private prescription or qualifying BMI alone does not establish NHS access.

There is more than one meaning of “eligible”. A private prescribing assessment, a national recommendation and today’s local NHS pathway answer different questions.

## Key points

- Use three separate headings in your notes: clinical suitability, the funded pathway and the local referral process. An answer to one does not settle the other two.
- A specialist weight-management assessment can consider several forms of support. Being referred to that service does not reserve a prescription for a particular medicine or dose.
- If you already pay privately, discuss the handover before relying on NHS supply. Keep assessment and monitoring records available so the receiving team can understand your current care.

## The current primary-care cohorts

NHS England’s phased plan prioritises people by BMI and specified conditions. Its table lists BMI 40 or above plus at least four qualifying conditions for cohort I; cohort II in 2026/27 covers BMI 35–39.9 plus at least four. The five conditions are hypertension, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease and type 2 diabetes. BMI thresholds are usually reduced by 2.5 for the ethnic backgrounds specified in the guidance. Later cohorts should not be treated as already open. Your local ICB and prescribing service confirm how this is being delivered. Lancashire and South Cumbria, for example, published a reduction in its threshold from 23 June 2026. That is a useful local implementation example, not proof that every practice uses the same referral process.

## Why the NICE recommendation looks different

NICE’s broader recommended group is adults with BMI at least 35 and at least one weight-related comorbidity, with appropriate lower thresholds for specified ethnic groups. England’s funding variation permits staged implementation. Being in that broader group does not automatically place someone in the current primary-care priority cohort. The practical question is therefore more specific than “Do I qualify for Mounjaro?” Ask which pathway the clinician is considering, whether the service is accepting referrals and what the next assessment would involve. An internet checklist cannot make that decision.

## Specialist care is a separate conversation

Ask whether a specialist weight-management referral is clinically appropriate. This is an assessment of your needs, treatment history and available service, not a guaranteed route to a chosen medicine. Specialist-service implementation and primary-care prioritisation should not be collapsed into one rule. Our provider guide explains where Oviva and regional examples fit. The provider’s NHS service may have different entry criteria and contact arrangements from any private offer shown on the same website.

## The care around the prescription

NHS England’s primary-care tirzepatide pathway includes structured dietary, activity and behavioural support alongside monitoring. Before a programme starts, establish who delivers that support, how reviews are arranged and which team answers questions. Which service owns the referral and prescription? What information is needed before an assessment? How will appointments work around your access needs? Who should you contact if your health or circumstances change? What support can you use while waiting?

## Already paying privately?

Keep a record of the medicine, prescribed dose, previous assessments and monitoring. A private prescription does not itself establish NHS funding entitlement. Discuss any proposed handover before stopping, switching or relying on a new supply route. If private cost is part of that conversation, Monj is Med Easy’s reference for Mounjaro price comparisons . Compare the prescribed product and repeat terms; a first-order saving is not a reason to change a clinical plan.

## Questions to ask

- Which named NHS Mounjaro pathway is being considered for me, and is it primary care or specialist care?
- Which current local policy and priority group will the receiving service use when it reviews the referral?
- What evidence of my health conditions, previous treatment and monitoring does the referrer need to include?
- Who would arrange the dietary, activity and behavioural support alongside any prescription, and how would I contact them?
- If NHS treatment is not offered now, what support is available and how can I check for a relevant pathway update?

## Frequently asked questions

### Does meeting NICE’s criteria mean my GP can prescribe now?

It does not establish immediate access through every service. England’s phased implementation and local arrangements must also be considered. Ask which pathway your practice is using and whether your referral would be assessed there. Keep the policy name with the answer so you can distinguish national guidance from local delivery.

### Can I move an existing private Mounjaro prescription onto the NHS?

A private prescription is useful evidence of current treatment, but it does not itself create NHS funding eligibility. Ask the proposed NHS service to assess the route and responsibilities. Agree who will prescribe and monitor treatment before relying on a handover; a letter being sent is not confirmation that it has been accepted.

### What should I do when an online eligibility checker gives a different answer?

Check which nation, date and pathway the checker describes. It may be summarising a licence, a broad recommendation or a local priority cohort. Bring the difference to your clinician as a question, with the source link, rather than treating either automated result as an individual clinical or funding decision.
