Hospital records may need a separate request to the hospital. Specify whether you need online visibility, a copy or a correction, and identify the dates and documents involved.",

"The app’s visible information is not necessarily the whole historical file. Ask about access settings and older entries before assuming that records no longer exist.",

Write down the approximate date and the organisation involved. That is more useful to the practice than simply reporting that “the app is wrong”. NHS England Digital explains GP records in the NHS App , including medicines, notes, results and documents. The requirement for online access concerns new information; historical entries are a separate question. These arrangements describe GP practices in England. If your care is elsewhere in the UK, ask your own practice which record-access service it uses.",

NHS England Digital’s practice guidance explains that surgeries control record visibility and historical access. The NHS App support team cannot amend your clinical record. If someone is pressuring you to reveal records, tell the practice using a contact method that is safe for you. "answer": "Historical visibility may depend on the access the practice has enabled. Ask the surgery about the period you need and whether it can provide online access or a copy. "answer": "The GP practice controls its clinical record; app support cannot amend it. Contact the surgery with the entry date and the detail you believe is inaccurate. If the information originated elsewhere, ask which organisation should review it. Keep the request and response so you can understand what was checked and changed.",

"answer": "Not necessarily. The NHS explains that hospital results need to have been sent to the GP to appear in that GP record. Ask the requesting hospital team how the result will be communicated and who will explain it. Online visibility and the clinical follow-up plan should be checked separately.",
